Transportation Feedback Form · Effective October 30, 2026

This form is not for emergencies. If a student is missing, hurt or in danger, or anyone is at immediate risk, call 911 right away. This form is not monitored around the clock.

These Terms of Use apply to the Fairfield Public Schools transportation feedback form ("the form"), run by the Fairfield Public Schools Transportation Department ("the district", "we", "us"). By checking the box and submitting the form, you agree to these terms and to our Privacy Policy.

1. What the form is for

The form lets parents and guardians, staff and community members share complaints, concerns and other feedback about student transportation in Fairfield Public Schools, such as bus service, schedules, stops, safety and drivers.

2. Not for emergencies

Do not use this form to report an emergency or anything that needs an immediate response. Call 911. For urgent, non-emergency problems on a school day, contact your school directly.

3. What you agree to

When you use the form, you agree to:

4. What happens with your feedback

5. Not a formal complaint or legal notice

This form is for transportation feedback. It does not replace the district's formal complaint procedures. If your concern involves discrimination, harassment, bullying, Title IX or a student's civil rights, please also follow the district's formal process by contacting your school's principal or the district office. A submission through this form is not a notice of a legal claim against the district.

6. Misuse

We may set aside submissions that break these terms, block further submissions from people or networks that misuse the form, and report threats of violence or other possible crimes to the police.

7. Your privacy

Our Privacy Policy explains what we collect, including technical details such as your IP address, and how we use and protect it. We never sell your information, and we do not share your contact details outside Fairfield Public Schools except as the Privacy Policy describes.

8. Availability

We try to keep the form available and working, but it is provided "as is" and may sometimes be unavailable. If the form isn't working, email Transportation@fairfieldschools.org.

9. Limits on liability

To the extent allowed by law, the district is not responsible for losses that result from using, or being unable to use, the form. Nothing in these terms waives any immunity or protection the district has under state or federal law.

10. Changes to these terms

We may update these terms. The new version will be posted here with a new effective date, and applies to submissions made after that date.

11. Governing law

These terms are governed by the laws of the State of Connecticut.
